Montana SDMI HCBS Waiver

Trias Pathways™ for SDMI Waiver Participants

Trias Coaching is approved to provide Life Coach services for adults participating in Montana’s Severe Disabling Mental Illness Home and Community-Based Services Waiver when coaching is included in the member’s authorized plan.

Trias Pathways supports person-centered goals, practical daily-life growth, confidence, accountability, and purposeful action in coordination with the member’s authorized support team.

Coaching begins with assessed needs and the goals established through the member’s authorized planning process.

Life coaching may support motivation, confidence, accountability, communication, and skills for greater independence and purposeful living.

Connected to the support team

Participation depends on waiver eligibility, assessed need, service authorization, provider availability, and coordination with the member’s case-management team.

SDMI Life Coach services do not provide medical care, psychotherapy, diagnosis, emergency response, or clinical treatment and may not duplicate other authorized services.

Members, case managers, and referral partners may contact Trias Coaching to confirm current availability and the appropriate referral or authorization pathway.

Guided experiences for awareness, alignment, and action

Trias Coaching offers guided programs and workshop experiences designed to help participants deepen awareness, reconnect the four MEPS dimensions, and turn insight into intentional next steps.

Format, timing, enrollment, and availability may vary and are confirmed through inquiry.

Take Flight Program

A six-session pro bono coaching pathway that helps participants clarify direction, strengthen whole-person alignment, and translate awareness into purposeful action.

May MEPS Challenge

A free annual group experience designed to help participants reconnect with the Mental, Emotional, Physical, and Spiritual dimensions through guided reflection and intentional action.

Four Systems Workshop

An interactive workshop for understanding the four MEPS dimensions, recognizing where life feels out of sync, and creating grounded, values-aligned next steps.

Trias Coaching offers nonclinical coaching, education, and supportive guidance for adults.

It does not provide medical care, psychotherapy, diagnosis, emergency response, or clinical treatment.

When a person’s needs fall outside the scope of coaching, Trias supports appropriate next steps and connection with qualified professional or emergency resources.
